SUBMITTING PROOF OR EVIDENCE OF A CORRECTIVE ACTION
INSTRUCTIONS
Instead of appearing in court on the date and time scheduled on your summons, you have the option of submitting proof or evidence of your corrective action through this portal. If you decide to proceed through this portal, you will not be required to appear in court but you waive your right to appear for a hearing in person, to request a trial, to question witnesses, and/or to object to any evidence. By submitting the information below to the court within five (5) days of your scheduled hearing, this constitutes your appearance in this matter and your authorization to verify the information submitted herein.
The court will review the summons and your statement and proof submitted below and render a decision. The penalty, if any, will be based on the facts of the case and your driving record. You will be notified of the court’s decision within thirty (30) days.
